A warm handoff involves connecting a client with a clinician or specialist (such as a psychologist, psychiatrist, or behavioral specialist) to ensure continuity of care. This process establishes a thorough understanding of the client's past, present, and future goals, providing a comprehensive knowledge base. At Transition House, we facilitate warm handoffs to specialists and clinicians, working closely with clients daily to guide them through their transitions and towards greater emotional and domestic stability.

He specializes in ensuring that there are no barriers due to age, disability, or other biases. Discrimination in these areas is unfortunately common, but we stand out as a one-stop shop and an additional advocate when needed to ensure fair and equal treatment for all.
We do not accept insurance at this time; our services are currently self-pay. You may qualify for support, especially if you are dealing with a respite situation or an emergency that does not require hospitalization or a crisis center.
YES. We can organize mental health services both in-home and in a new residence. This includes assisting with finding and hiring specialists (self-pay) or using your health insurance to secure a licensed caregiver to monitor your loved one in any chosen setting.
Absolutely! We have extensive connections and resources with care facilities and assisted living communities. We can provide a variety of options, referrals, and even assist in coordinating specialized services, including mental health support, if desired. All of this is in addition to the standard services and fixed expenses associated with assisted living facilities.
The cost of an assisted living facility typically ranges from $4,000 for a shared room to $8,000 for a private room. Additional services, such as mental health support, transportation, recreational activities, and visits from clinicians and specialists, are not included in this base cost. Our team is here to assist you in arranging these additional services as needed.

We offer tailored support for independent living clients who require assistance with mental health services or housing but do not need full care management or an assisted living facility. Here’s how we can help:
Medication Management: We assist with medication management at a new home or residence that we help you find.
Daily Integrative Services: For qualified individuals, we can set up daily integrative services in an apartment. Clients must be able to take their medication independently, but Transition House can cover other expenses.
Families pay an agreed-upon monthly amount that covers:
Rent and utilities
Medication management (without assistance)
Strength training and coaching
Recreational activities
Monthly assessments to gauge progress

Allowing support or therapy animals can vary based on the situation. For the elderly entering an assisted living facility, it can be more challenging. However, for individuals under 55 moving into an apartment or other settings, it's usually permitted. Each case is unique, so we assess and make accommodations on a case-by-case basis.
In assisted living facilities, meals are typically prepared by nurses and staff. For those in apartments or homes with independent residential services, groceries are bought, or food is ordered. Our goal is to teach or re-teach clients how to cook, clean up after meals, do dishes, and manage other aspects of independent living without clinical support.
